I've tried to rule out every factor, but still cant get rid of noise (i know it will be something silly, so sorry in advance)
This is my (simplified) setup (atm)

1 Humbucker (Jazz) in neck of Les Paul (2 conductor wiring)
Pos wire to pos lug in output jack, Neg to neg lug in output jack. Wire from bridge to neg jack.

Noise is constant, and slightly louder when i take my hands off guitar.

What am i doing wrong?

And no, its not my cable, not what i'm plugging into, etc. Gawd i feel like an idiot (and i have a gig tonite!)

Sounds like a ground issue, I have the same problem with my telecaster, but It's not noticeble unless I take my hands off, otherwise it's pretty quiet for true singles.

I agree. Looks like you're gonna have to go through your wiring again, maybe try and find a better ground for that pickup.
